1. Services Provided

We offer IV hydration therapy, vitamin infusions, wellness injections, and related services. Our services are intended to support health and wellness but are not a substitute for medical treatment. You should consult with a licensed medical professional before receiving IV therapy.

2. Eligibility for Services

By booking an appointment or receiving services, you confirm that you:

• Are 18  years or older (or have a legal guardian’s consent if under 18)
• Have provided truthful and accurate medical information
• Do not  have any contraindicated medical conditions (as determined by our licensed      providers)

3. Medical Disclaimer

Our services are provided by trained medical professionals; however:

• We do  not diagnose, treat, or cure medical conditions
• We do  not guarantee specific health outcomes
• You  assume full responsibility for your health decisions

4. Appointments, Cancellations & Refunds

A. Booking & Payment

• Appointments      require advance booking and payment at the time of service
• We      accept cash, credit/debit cards, and other approved payment methods

B. Cancellation Policy

• Cancellations  or reschedules must be made at least 12      hours in advance
• Cancellations made less than 12 hours before an appointment may be subject to a cancellation      fee
• No-shows  will be charged the full service fee

C. Refund Policy

• No  refunds will be issued once a service      has been provided
• If you  are dissatisfied with a service, please contact us to discuss resolution      options

5. Risks & Liability Waiver

By using our services, you acknowledge and accept that:

• IV      therapy involves potential risks and side effects, including but      not limited to allergic reactions, bruising, and discomfort
• You voluntarily      assume all risks associated with IV therapy
• You      release Whiting Health LLC DBA IV Wellness AZ and its employees      from any liability arising from service use, except in cases of      gross negligence

6. Privacy & Confidentiality

We respect your privacy and comply with HIPAA regulations. Please refer to our Privacy Policy for details on how we collect and protect your personal and health information.

7. Code of Conduct

To maintain a safe and professional environment, clients agree to:

• Treat      staff and other clients with respect
• Provide      honest and accurate medical history
• Refrain      from disruptive or inappropriate behavior

Failure to comply may result in refusal of service.

8. Intellectual Property & Website Use

All content on our website (logos, text, images) is protected intellectual property. You may not copy, distribute, or use our content without permission.

9. Governing Law

These Terms and Conditions are governed by the laws of the state of Arizona.Any disputes shall be resolved through mediation or arbitration before legal action is pursued.
